The format you pick changes how easy the answers are to read later. A few common choices for feedback-style questions:

Multiple choice and rating scale questions are the easiest to scan across a large group. Save short answer questions for when you genuinely want someone's own words, since they take longer to read through afterward.
Once people have started responding, pull up the answers from the Reports & Stats button in your sign up's Administrator's Toolbar. Choose List of Sign Ups for Export to Excel as the report style, then show all fields or just the ones you need. Your survey answers appear under Custom Fields.
Click Create Report to see the responses in your account, or Export Data as CSV File to download them as a spreadsheet you can open in Excel or any spreadsheet tool.
